This is a detailed questionaire that asks for a lot of background information about the concerns you are seeking help with, and will probably take you about an hour to fill out.  Please do this as thoughtfully as possible.  I will also need your contact and insurance information from this form, as well as your formal agreement for treatment for me to be able to work with you. It is very important that you fill this out in some detail prior to seeing me, and bring it with you to our initial meeting - this will greatly assist in my understanding of what may be most helpful for you.
